Doula Gold Standard
The Doula Gold Standard Designation Raises the Bar for Collaborative Maternal Care in Louisiana’s Hospital Systems.
Overview
The Doula Gold Standard designation promotes and recognizes birthing facilities that demonstrate a clear commitment to fostering respectful, collaborative, and inclusive partnerships with doulas. It is awarded to facilities that implement the policies and practices listed in the Doula Gold Standard Checklist.
By outlining a tiered designation system based on the implementation of evidence-informed policies and practices, this initiative aims to improve the birthing experience and outcomes for families by supporting environments where doulas are fully integrated members of the care team.
The Doula Gold Standard aims to improve maternal and infant health outcomes by increasing patient access to collaborative doula care, reducing preventable morbidity and mortality, decreasing racial disparities in maternal care, and improving the adoption and sustainability of evidence‑based practices that improve quality of care and patient experience.
About
In 2022, Louisiana allowed Medicaid managed care plans and private insurers to cover and reimburse for doula services. These policy shifts increased access to doula services for birthing people. However, further efforts are needed to fully integrate doulas into hospital care teams to enhance collaboration throughout prenatal and postpartum care. In supportive clinical environments, doulas function as key links between care teams and families during critical moments. Expanding access to community-based and Medicaid-covered doula programs in Louisiana has the potential to reduce preventable maternal deaths, mitigate racial disparities, and strengthen outcomes in underserved parishes.
In response to these legislative changes, in 2025, LPHI’s Doula Collaboration Community of Practice (CoP) convened community-based doulas, birth workers, and clinical providers to establish collaboration between doulas and care teams. Communities of Practice are designed to provide a structured yet flexible learning environment that supports peer connection, shared problem-solving, and practical application of evidence-based strategies. Early accomplishments of the Doula Collaboration CoP included developing a shared definition of respect.
Shared definition of respect:
Respect for patients, doulas, and medical staff is the concept that all people acknowledge the collective goal of a good and healthy birth. Showing respect for all individuals is recognizing all roles of the care team, accommodating patient choices, communicating, and acting professionally. This concept applies throughout prenatal, labor & delivery, and postnatal care.
CoP participants expressed a strong desire for tangible policy solutions and strategies to improve respectful collaboration between doulas and care teams. Drawing on lessons learned from national partners that emphasize the importance of cultivating collaborative environments that support co-development of strategies for improved care coordination and respect across the care team, CoP participants developed The Doula Gold Standard, a Doula-led initiative that defines what collaboration looks like and calls for systems-level policy change to improve patient access and outcomes.
Doula Gold Standard Checklist
Building upon this definition of respect and acknowledging the expressed desire of CoP participants for tangible solutions and strategies to promote collaboration between doulas and other members of maternal care teams, the group developed The Doula Gold Standard.
Categories
Each Doula Gold Standard checklist item is categorized by one of the following categories:
(P) Policy/Procedure – Changes made to address regulations, procedures, and administrative actions that could foster greater collaboration between doulas and clinical care teams.
(T) Training – Structured activities that increase staff knowledge about doula care and related processes.
(E) Environment – Adjustments to the physical space or social context that enhance patient experience and support collaborative care.
(M) Materials – Supplies and resources that facilities can have available to support doula care and improve efficiency.
Tiers
The Doula Gold Standard is divided into three tiers of achievement with increasing requirements and complexity. By adopting a tiered structure and comprehensive categories, the Doula Gold Standard supports consistent implementation, meaningful collaboration, and transparent evaluation.
Tier 1 - The Essentials: The Essentials tier includes practices that establish a baseline inclusive environment for doulas. These practices are non-negotiables that should be implemented first to create a collaborative environment.
Tier 2 - A Step Above: This tier includes mid-level practices that require more time and effort than the first tier and may require staff buy-in or additional resources to implement.
Tier 3 - Above and Beyond: The Above and Beyond tier includes practices that provide the most inclusive and collaborative work environment for doulas.
